Some are free. I give my self an Apple Gift Card, which I then redeem,
then I have a balance on my iTunes account which I use to purchase apps
for my iPod touch and iPad. You can just use your credit card or bank
debit card.

All software where download via iTunes (App Store).
It works like other Apple Store in iTunes : you must be registered, you
must gave your credit card reference.
For free app, you won't be charge ; for not-free app you'll be charge
with tha amount indicate on the store...
Some app are free and propose in-app store (things you can buy from the
app itself, but it's the same process Apple's AppStore will charge).

Yes (it's not really easy to see, but you can registered without credit
card) ; but you need one to pay : it was the question.

Is that right? Can people download free apps without having an Apple
account?

you do not need a credit card to pay. you can use itunes gift cards.
