I see quite a lot of this on google, with no solutions.

I have tried two different HDMI adapters.

It worked for about half an hour, and then stopped.

Is there some config for whether the Ipad outputs the video via HDMI
or not?

I know the TV is OK because I can test it with a camera.

The firmware is the very latest - the update made no difference.

If the hardware is broken, can they test that in the Apple shop?

Not a lot of takers on this one....

I have got it working in some contexts. It is app dependent, it is
dependent on the order in which the Ipad v. the HDMI adaptor are
switched on/ plugged in. Whether the video is shown on both the Ipad
and the TV also depends on the app, but there is no config for this
anywhere. Various people have reported this kind of crap in the Apple
forums.
